Make sure you do not include the nominee’s last name or address in the submission field.

We know you have many accomplishments on the field, in school, and in the community. But speak with a parent or coach to help you decide which ones you’re most proud of. Then, tell us why you deserve to be SportsKid of the Year and give us a few quick highlights.

For example, if New Orleans Saints quarterback Drew Brees were eligible to be named SportsKid of the Year (too bad he’s too old!), this is what his form might look like:

I think I should be SKOTY because I overcame a tough start in the NFL and eventually become a Super Bowl champion. Even though I was never the biggest kid on the field, I worked hard and never gave up. My proudest NFL memory was working with my team to bring New Orleans its first championship. But I don’t take success for granted. I give back to my community. I’ve helped rebuild schools and have raised money for cancer research. My highlights include:

– As a QB for the Saints, I led them to win Super Bowl XLIV and was named Super Bowl MVP.
– I am the all-time single season passing yards leader.
– I helped New Orleans rebuild after Hurricane Katrina. I won the Walter Payton Man of the Year award for community service.
– I graduated from Purdue University and was an Academic All-America as a senior.
